The Orthopaedic Surgery Match Timeline: What to Do in MS1, MS2, MS3, and MS4.
Most applicants start preparing in third year. The ones who match their top programmes started in first year. Here is the full timeline.

Why the timeline matters more in ortho than any other specialty
Orthopaedic surgery is won over years, not months. The students who match HSS, Mayo, and Johns Hopkins did not begin preparing in MS3 when ERAS opened. They began in MS1 — identifying research opportunities, cultivating faculty relationships, and building a profile that would take three years to fully develop. The students who scramble in MS3 to patch a thin application are playing catch-up in a race that was already decided. This timeline is your roadmap. Use it.
MS1 — The Foundation Year
Your first priority in MS1 is to confirm that orthopaedic surgery is genuinely the right specialty for you. Shadow orthopaedic surgeons, attend ortho clinic, and spend time in the OR if possible. Specialty confirmation matters because every subsequent decision — research focus, elective selection, networking — flows from it. Once confirmed, begin building your research profile immediately. Identify faculty at your home institution who are actively publishing in orthopaedics. Introduce yourself. Ask to contribute to an ongoing project. A poster presentation submitted at the end of MS1 is a strong start. Protect your GPA — ortho programme directors pay attention to preclinical grades. And begin developing a clear roadmap: where do you want to match, what does your profile need to look like to be competitive there, and what needs to happen each semester between now and application season.
MS2 — The Profile Building Year
MS2 is when the gap between matched and unmatched applicants begins to open. The students who will match top programmes are submitting abstracts, accumulating research experiences, and building genuine relationships with orthopaedic faculty. Your research output should be accelerating — aim for at least one to two submitted abstracts or a publication in progress by the end of MS2. Step 2 CK preparation should begin in earnest — target a score of 255 or above, and ideally 260 or higher for top programmes. Begin your away rotation shortlist — identify three to five programmes where you would be happy to match and that are within your competitive range. Begin cultivating the faculty relationships that will generate your strongest letters of recommendation. Your LOR writers need to know you well — that takes time.
MS3 — The Application Year
MS3 is the most intense year of the ortho match process. VSLO opens in the spring and away rotation spots fill quickly — apply early. Your away rotations should be completed by October at the latest to allow time for letters to be submitted before ERAS deadlines. ERAS opens in the summer — your personal statement, CV, and application should be in draft form before away rotations begin, not after. Signal allocation is one of the most consequential decisions of your application — you receive a limited number of signals to express interest in specific programmes and how you allocate them materially affects your interview offers. Interview season runs October through January. Prepare thoroughly for every interview — your OrthoMatched mentor will run mock sessions specific to each programme on your list.
MS4 — The Closing Stretch
By MS4 the heavy lifting is done. Your focus shifts to rank list strategy and the transition to residency. The rank list is submitted in February — this is a strategic decision, not just a preference list. How you order programmes affects your match outcome in ways that are not always intuitive. Your OrthoMatched mentor will run a dedicated rank list strategy session to make sure every decision is deliberate. Match Day is in March. If you do not match, SOAP begins immediately — your mentor will be available throughout Match Week regardless of the outcome.
