Away rotations are auditions. The programme that hosts your away is far more likely to rank you. Here is how to choose strategically.

Away rotations are not optional in ortho
In most specialties, away rotations are helpful. In orthopaedic surgery, they are essential. Programme directors at top ortho programmes consistently report that sub-internship performance is one of the most heavily weighted factors in their ranking decisions. The reason is simple: ortho is a small world. Programme directors talk to each other. A strong away rotation at a programme does three things simultaneously — it gives you a direct audition for a spot, it generates a letter of recommendation from someone the programme director knows personally, and it signals genuine interest in that programme in a way that no signal on ERAS can replicate.
How many aways should you do
The AOA and CORD recommend a limit of four medical student rotations inclusive of home and away rotations. Most competitive ortho applicants do two to three away rotations strategically chosen to maximise their chances at target programmes. Doing more than three is generally not advisable — it raises questions about why you need so many auditions and limits time for other MS3 priorities. Two well-chosen aways at programmes where you have a genuine chance of matching are worth more than four mediocre aways at programmes out of your reach.
How to choose which programmes to rotate at
The single most important question to ask when selecting away rotation programmes is: would I genuinely be happy matching here? Away rotations are most effective when the programme is also on your realistic rank list. Rotating at a programme you would not rank highly is a wasted audition slot. The second question is: does this programme have a history of ranking away rotators? Some programmes use aways extensively as recruitment tools and regularly match students who rotated there. Others host aways as a courtesy but rarely rank rotating students. Your OrthoMatched mentor — who matched within the last three years — knows which programmes fall into which category. That intelligence is worth more than any publicly available information. The third question is: is this programme within my competitive range? Rotating at HSS when your Step 2 is 245 and you have two publications is unlikely to result in a match there. Choose programmes where your profile is genuinely competitive. A successful away at a strong programme that is within your reach is worth more than an unsuccessful away at the most prestigious programme in the country.
How to perform on your away rotation
An away rotation is a month-long job interview. Programme directors and residents are evaluating whether they want to work with you for five years. The students who perform well on aways share common traits: they come prepared, they are reliable and enthusiastic without being overbearing, they know their anatomy and relevant surgical approaches, and they make the residents' lives easier rather than harder. The VSLO application and timing
Away rotations in orthopaedic surgery are applied for through the Visiting Student Learning Opportunities portal, known as VSLO. Applications typically open in the spring of MS3 for rotations in the summer and fall of the same year. Competition for spots at top programmes is fierce — some programmes receive 10 to 15 applications for every available rotation slot. Apply early, write a strong personal statement tailored to each programme, and have your home programme coordinator confirm your good standing before applying.
